Malpractice Insurance<\/strong><\/h6>\n

Malpractice insurance coverage is provided by the Health and Hospitals Corporation (HHC) of the City of New York<\/p>\n

Education Benefit<\/strong><\/h6>\n

CIR provides: $750 Professional Education Plan (PEP) each year
\n$1,500 Continued Learning Program (every 3 years)
\n$3,000 QI\/PS Quality Improvement Scholarship<\/p>\n

Health Coverage<\/strong><\/h6>\n

Choice of health plans provided by HHC. Medical Plan, GHI-Comprehensive benefits program of Health Insurance Plan (HIP) or Group Health Incorporated (GHI) plus Blue Cross Hospital Benefits. All Plans include maternity benefits and benefits for dependent children.<\/p>\n

Prescription Coverage<\/strong><\/h6>\n

CIR provides: $500 yearly for member and $500 yearly for each dependent<\/p>\n

Optical Coverage<\/strong><\/h6>\n

CIR provides: One free eye exam yearly for each dependent and one free pair of lenses and set of frames yearly for each dependent (up to $225)<\/p>\n

Dental Coverage<\/strong><\/h6>\n

CIR provides two plans through Guardian: Managed Dental Guard (an in-network plan) and Dental Guard Preferred (an out-of-network plan)<\/p>\n

Life Insurance<\/strong><\/h6>\n

CIR provides: $150,000 for member -- $20,000 for spouse<\/p>\n

Disability Coverage<\/strong><\/h6>\n

CIR provides:
\nShort Term Disability -- up to $875 a month up to 90 days
\nLong Term Disability -- 70% of annual salary up to $3,500 per month<\/p>\n
