<\/span>His doctor has him on <span class=\"SpellE\">Actos<\/span> &ndash; the patient tried her husband&rsquo;s medication twice and says it was &ldquo;a good pill.&rdquo;<span style=\"mso-spacerun: yes;\">&nbsp; <\/span>What would you recommend?<\/p>\n<p class=\"MsoNormal\"><span style=\"mso-tab-count: 1;\">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; <\/span><\/p>\n<p class=\"MsoNormal\" style=\"text-indent: 0.5in;\"><em>-What evidence is there to support recommendations for a particular diabetic diet?<\/em><\/p>\n<p class=\"MsoNormal\"><em><span style=\"mso-tab-count: 1;\">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; <\/span>-Is there evidence to support one oral therapy over another?<\/em><\/p>\n<p class=\"MsoNormal\"><em>&nbsp;<\/em><\/p>\n<p class=\"MsoNormal\">3.<span style=\"mso-spacerun: yes;\">&nbsp; <\/span>The patient is a 55 year-old mixed race Hispanic man who has had diabetes for eight years.<span style=\"mso-spacerun: yes;\">&nbsp; <\/span>Initially he responded to recommendations for diet and exercise &ndash; he lost five pounds and his glucose normalized.<span style=\"mso-spacerun: yes;\">&nbsp; <\/span>Then two years later, despite continued efforts, his glucose rose and he began taking <span class=\"SpellE\">metformin<\/span>.<span style=\"mso-spacerun: yes;\">&nbsp; <\/span>After increasing the dose to 1000 mg bid, the glucose normalized again.<span style=\"mso-spacerun: yes;\">&nbsp; <\/span>Then after two years, the glucose began to climb again.<span style=\"mso-spacerun: yes;\">&nbsp;
