Due to
\nthe hormonal and hemodynamic alterations inherent in the
\nsurgicalexperience, acute renal failure is common during
\nthe perioperative period.Acute renal failure occurs in
\n5% of hospital admissions, and the surgicalsetting is
\nthe second most common cause of inpatient acute renal failure.
\n<\/sup>Because this setting has the highest mortality for acute renal
\nfailure,recognition of high-risk patients is essential
\nfor careful monitoring andprophylactic measures.
\nPatients with chronic renal insufficiency, elderly
\n<\/sup>patients, jaundiced patients, diabetics, and those undergoing
\ncardiac oraortic surgery are at greatest risk for
\nperioperative acute renal failure.Patients with severe
\nchronic renal failure or end-stage renal disease areat
\nsignificant risk for development of complications during the
\n<\/sup>perioperative period, due both to renal and nonrenal reasons.
\nHyperkalemia,infections, arrhythmias, and bleeding
\ncommonly occur in these patientsduring the perioperative
\nperiod. This population has a reasonable surgical
\n<\/sup>mortality for both general and cardiac surgery, but the
\nextremely highmorbidity warrants careful perioperative
\nmonitoring and care. <\/span>
