Planned Visits<\/span><\/strong><\/p>\n

\u00a0<\/span><\/p>\n

\u00a0<\/span><\/p>\n

For this ambulatory block you each have a session titled: Planned Visit.<\/span><\/p>\n

This is a special session that has been reserved for you to see a patient in a different way.\u00a0 <\/span>In following the \u201cChronic Disease Model of Care\u201d \u2013 this is an appointment which is meant to be more patient-centered and focused on the needs of the patient.\u00a0 <\/span>This means that the visit will be planned for \u2013 depending on the needs of the patient.\u00a0 <\/span>The patient will prepare and you will also prepare for the visit.<\/span><\/p>\n

\u00a0<\/span><\/p>\n

You may choose a patient with Diabetes whom you think would benefit: Pick a patient from your diabetes registry or one that you have seen earlier in the month.\u00a0 <\/span>For example:<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient who is not controlled<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient who is not following up with appointments<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient who you feel has obstacles to adherence with medication regimen where spending more time to sort that out may be useful<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient who needs to see the dietician and\/or diabetic educator in conjunction with your appointment.<\/span><\/span><\/p>\n

\u00a0<\/span><\/p>\n

Alternatively<\/span><\/strong>, you may choose a patient who you think could benefit from a more comprehensive Geriatric Assessment.<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient who has had a recent fall<\/span><\/p>\n

\u00a0<\/span>\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient who appears at risk for functional decline \u2013 for any one of a variety of reasons<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient with possible or definite cognitive impairment<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient where family seems to be excessively burdened by care and not following through on treatment plans<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>An elderly patient who appears depressed<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>An elderly patient on multiple medications where medication issues may be an important issue<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient where you think the social worker could be of benefit to discuss family support, home care, respite programs<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>A patient who needs to discuss advance care planning with you.<\/span><\/span><\/p>\n

\u00a0<\/span><\/p>\n

You would ask a patient to return to your Planned Visit session and follow the steps in the Geriatric Assessment powerpoint<\/span> presentation:<\/span><\/p>\n

\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span>As part of your visit you may perform: <\/span><\/p>\n

Functional Assessment \u2013 via ADL and IADL scale<\/span><\/p>\n

Cognitive Assessment \u2013 via MMSE or MIS, or minicog<\/span><\/p>\n

Assessment of Mood with the Yesavage Depression screen<\/span><\/p>\n

Assessment of Gait \u2013 Get up and Go test<\/span><\/p>\n

Medication Review with all meds \u2013 OTC and prescribed \u2013 patient should bring in all meds- \u201cthe brown bag visit\u201d \u2013 tell them to bring in meds<\/span><\/p>\n

Social Assessment \u2013 interview of caregivers and family<\/span><\/p>\n

Advance Care Planning and Health Care Proxy discussion<\/span><\/p>\n

Document these visits as a usual visit but inform the attending supervising that this is your planned visit patient.<\/span><\/p>\n
