Instructions:
1) A letter or script indicating the reason for referral, pt’s diagnosis, and past/current treatment history.
2) Once a pt is provided with either a letter or script they can call to schedule their own appointment.
3) The pt must have their insurance information at hand when calling to schedule an appointment.
