A gynecologic examination is
important to establish any evidence of assault. The exam may not be optimal in this situation because some time
has passed and the patient likely would have showered causing further loss of
evidence. However hymen rupture,
lacerations or bruising may be helpful.
Additionally, STD’s including GC, gonorrhea, and HIV, should be ruled
out. It would be too early for any
signs of pregnancy from this assault to be present. The patient should be examined generally for signs of trauma and
mental distress.
The more sensitive pregnancy test, serum ß-HCG, would not be positive until
a week after conception. Therefore,
aside from ruling out any prior pregnancy, it would not be of any value in
evaluating patients for emergency contraception.
