Nurses Residence Psychiatry Referral

The number to call for the referral is 3-3462.

Ms. Judy Solomon at 3-5039 is the administrator in charge of the clinic and she invited us to call her regarding appointments if there is a problem.

As per Dr. Inglaterra, doctor or PCA can call with diagnosis. Psychiatrist in Building 4 will review your Quadramed note, which should include the following:

• Diagnosis (you should add the diagnosis to the problem list)
• Reason for referral
• Medications that patient is using for this problem (if any)

Priority is given to Metroplus and the uninsured, but please make the referral if in doubt.
