1.  Your patient is a 51 yo woman who complains
of burning stomach, chest discomfort, and acid taste in her mouth half hour
after eating.  She has tried
over-the-counter antacids and H2-blockers with some but incomplete relief.  There are no other complaints and ROS and
exam are negative.  Labs are normal.

 

Do you do further diagnostic testing?  If so what? 
What are the values of UGIS, endoscopy, trial of treatment, helicobacter
pylori testing?

What are the values of various treatments?

 

2.
Your patient is a 51 yo woman who complains of epigastric discomfort somewhat
related to meals.  She has tried
omeprazole with incomplete relief.  Exam
is normal.  Labs are normal except for
serum helicobacter pylori titer (IgG) positive. 

 

Do you do further diagnostic testing?  If so what? 
What are the values of UGIS, endoscopy, trial of treatment?

What are the values of various treatments?

Your patient is a 31 yo woman who complains of hemorrhoids – intermittent
rectal pain and bleeding with sensation of external rectal mass.  She has tried Preparation H suppositories
without much relief.
