A Brief Intro

Patient Centered Medical Home is a descriptor of a practice where the patient has access to high-quality continuity, physician- directed care that is coordinated across the healthcare system.

To comply with PCMH, Jacobi Medical practice has been reorganized into teams consisting of physician providers, NP walk-in providers, PCAs, nurses, social workers and clerical staff. Besides the clinic visit, various workers will contact the patient prior to the visit and also between visits, as needed. There is after-hours and weekend phone consultation availability 365 days a year.

Our practice will transition to electronic prescribing with the initiation of the new EPIC electronic medical record.
